Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes, confer with physicians to revise Plans of Care, and provide instruction to restore function or teach compensation measures. Review retraining programs, consult with vocational team members, and design community reintegration activities. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations, and submit discharge summaries. Participate in care coordination, discharge planning, and team education meetings.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
