Job Summary
Assess patient daily living skills, administer diagnostic tests, and develop therapeutic retraining programs with measurable objectives. Confer with physicians to revise Plans of Care, provide treatment and instruction to improve strength and function, and design community reintegration activities. Coordinate discharge planning, recommend adaptive equipment, and train staff on hygiene and grooming skills. Document care observations, implement interim orders, and report progress to the physician and interdisciplinary team. This role supports patients recovering from injury, illness, or surgery in their homes.
Required Qualifications
- Degree from an accredited Occupational Therapy Program
- Current and unrestricted OT licensure
- Current CPR certification
- A valid driver's license, auto insurance, and reliable transportation
Desired Qualifications
- A minimum of six months of occupational therapy experience preferred
- Home Health experience a plus
