Job Summary

Occupational Therapist Home Health role providing in-home rehabilitation and functional retraining to help patients regain strength, mobility and independence. Responsibilities include assessing daily living/work-related skills, developing therapeutic retraining programs with measurable objectives, administering and interpreting functional assessments, coordinating with physicians and the interdisciplinary team to develop and revise the Plan of Care, delivering therapeutic treatment per physician orders, documenting care observations, contributing to care coordination and discharge planning, designing community reintegration activities, and training staff and family members to support the retraining program. The role also involves supervising OT Assistant and Home Health Aide performance, and ensuring proper documentation of evaluations, treatment plans and discharge summaries. Requires OT licensure, CPR, a valid driver’s license with insurance, and means of reliable transportation. TB screening is part of Humana’s TB program for this patient-facing position. Pay ranges between $85,400 and $117,500 per year depending on location and experience.

Required Qualifications

  • Current and unrestricted OT licensure
  • Current CPR certification
  • Valid driver's license, auto insurance and reliable transportation

  • TB screening program participation required
