Occupational Therapist Home Health

Assess and screen patients' daily living and work-related skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, then confer with physicians to revise treatment plans. Provide therapeutic instruction to improve strength, coordination, and range-of-motion while teaching compensation measures. Consult with vocational team members to implement vocationally oriented plans and design community reintegration activities. Recommend adaptive equipment for residence or working environments and train staff and family members on dressing, feeding, and hygiene skills. Supervise Occupational Therapy Assistants and Home Health Aides, documenting care observations, interventions, and evaluations daily. Submit evaluation plans and discharge summaries to supervisors and care management staff.
