Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, provide treatment and instruction to restore strength and coordination, and teach compensation measures. Review retraining programs, consult with physicians and team members to revise Plans of Care, and design community reintegration activities. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and interventions, and coordinate discharge planning. Submit evaluation plans and discharge summaries to management. Work 40 weekly hours at a pay rate of $49.29 to $67.82 per visit.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
