Job Summary
Provide occupational therapy services in Home Health to help patients regain strength, mobility and independence in their homes. Assess and screen patients’ daily living/work-related skills, develop therapeutic retraining programs with measurable objectives, administer and interpret functional tests, collaborate with physicians and the care team to update the Plan of Care, deliver therapeutic treatment per physician orders to improve/restore function, document care observations, and participate in care coordination, discharge planning, and staff/family training to support home-based rehabilitation and community reintegration.
Required Qualifications
- A minimum of six months of occupational therapy experience preferred
- Home Health experience a plus
- Current and unrestricted OT licensure
- Current CPR certification
- Good organizational and communication skills
- A valid driver’s license, auto insurance, and reliable transportation are required
