Job Summary
Occupational Therapist in Home Health delivering in-home rehabilitation services. Assess daily living/work-related skills and develop therapeutic retraining programs with measurable objectives; administer and interpret functional tests to assess patients' level of function; collaborate with the physician and care team to develop and revise the Plan of Care; provide therapeutic treatment to improve/restore strength, coordination, range of motion and function; design community reintegration activities; consult with vocational team members to align plans with patient needs; report patient status to physicians and family; supervise OT Assistant and Home Health Aide performance; document care observations and plans; participate in care coordination and discharge planning; ensure adaptive equipment is designed for client environments; drive patient care to home settings; TB screening and driver-safety requirements are part of the role. Required qualifications include current and unrestricted OT licensure, CPR, and a valid driver's license with auto insurance.
