Occupational Therapist, Home Health

Provide home-based occupational therapy to patients in Chesterfield County. Assess patients’ daily living/work-related skills and develop measurable retraining programs; administer and interpret functional tests to support physician evaluations. Collaborate with the physician and rehabilitation team to develop/revise the Plan of Care; deliver therapeutic treatment to improve/restore strength, coordination, range of motion, and function; teach compensation strategies. Review and expand retraining programs to align with behavioral goals and rehabilitation guidelines; coordinate with vocational team members to design community reintegration activities. Document patient status, progress, and discharge planning; supervise OT Assistants and Home Health Aides; contribute to care coordination and family education. Design equipment recommendations to improve home or work environments and train staff/family to carry out retraining activities, including activities of daily living such as dressing, feeding, grooming and hygiene. Communicate findings to the care team and participate in interdisciplinary planning to meet patient needs.
