Job Summary
Occupational Therapist – Home Health position at CenterWell/Home Health (Humana) delivering in-home rehabilitation services. Responsibilities include assessing and retraining daily living/work-related skills, administering and interpreting function-related diagnostics, collaborating with physicians and the care team to develop and revise Plans of Care, providing therapeutic treatments to improve strength, coordination, ROM and function, expanding retraining programs, coordinating care and discharge planning, designing community reintegration activities, recommending adaptive equipment, training staff and family members, supervising OTAs and Home Health Aides, and thoroughly documenting patient care observations. Requires current OT licensure, CPR certification, and a valid driver’s license with reliable transportation. Schedule is 40 hours per week with a base pay range of $85,400–$117,500 per year; TB screening and driver-safety program participation are included as part of the role. Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
