Job Summary
Occupational Therapist (OT) needed for CenterWell Home Health to deliver in-home rehabilitation services. Responsibilities include assessing daily living/work-related skills, developing and administering therapeutic retraining programs, coordinating with physicians and care teams, providing therapy to improve strength, ROM, and functional performance, documenting progress, planning discharge and care coordination, and training staff and family members in retraining programs. Emphasis on home-based rehabilitation, interdisciplinary collaboration, and designing adaptive equipment or compensatory strategies to enhance client independence. Requires OT licensure, CPR, and reliable transportation; pay range $85,400–$117,500 annually; full-time schedule.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation.
