Job Summary
Occupational Therapist Home Health provides in-home rehabilitation services to help patients regain strength, mobility and independence. Responsibilities include assessing daily living/work-related skills, developing retraining programs with measurable objectives, interpreting functional tests, collaborating with physicians and the care team, delivering therapeutic treatment per physician orders, documenting care observations, participating in care coordination and discharge planning, and designing community reintegration activities. Supervises OT assistants and home health aides, and educates staff and family on retraining programs (e.g., dressing, feeding, grooming, hygiene). Requires OT licensure, CPR, and a valid driver’s license with reliable transportation.
