Occupational Therapist Home Health

Occupational Therapist Home Health provides in-home rehabilitation and functional retraining to help patients regain strength, mobility and independence. Responsibilities include assessing daily living/work-related skills, developing therapeutic retraining programs with measurable objectives, administering and interpreting functional tests to support physician evaluation, collaborating with the physician and interprofessional team to develop and revise the Plan of Care, delivering therapeutic treatment per physician orders to improve/restore function, documenting patient progress, coordinating care and discharge planning, and supervising/educating staff and family members to implement retraining programs. Focus is on helping patients perform activities of daily living with confidence, coordinating with the interdisciplinary team to design community reintegration activities, and ensuring proper documentation and communication of care. Requires current OT licensure, CPR certification, and a valid driver’s license with reliable transportation. TB screening is required; full-time schedule with pay range $102,500 - $141,000 per year.
