Job Summary
Occupational Therapist in Home Health delivering therapeutic retraining and rehabilitation services in patients' homes. Assess/screen daily living/work-related skills, develop measurable retraining programs, interpret function tests, participate in Plan of Care development with physicians, provide/verify therapy to improve strength, coordination, ROM and functional abilities, review and expand retraining plans, collaborate with vocational team members, report progress to physicians and care management, participate in care coordination and discharge planning, design community reintegration activities, recommend adaptive equipment, train staff and family in retraining activities, supervise OT Assistant and Home Health Aide performance, and thoroughly document patient care observations and plans. Requires degree from an accredited OT program, OT licensure, CPR, a valid driver’s license, good organizational/communication skills, and Home Health experience preferred.
Required Qualifications
- Degree from an accredited Occupational Therapy Program
- A minimum of six months of occupational therapy experience preferred
- Home Health experience a plus
- Current and unrestricted OT licensure
- Current CPR certification
- Good organizational and communication skills
- A valid driver’s license, auto insurance, and reliable transportation are required
