Job Summary
Occupational Therapist in home health to assess daily living and work-related skills, develop therapeutic retraining programs with measurable objectives, administer and interpret function tests in patients’ homes, collaborate with physicians and rehab team to plan and revise care, provide in-home therapy to improve strength, coordination, ROM and functional independence, design community reintegration activities, coordinate with interdisciplinary team, supervise OT Assistants and Home Health Aides, document patient care observations and treatment plans, and participate in care coordination and discharge planning.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation.
