Occupational Therapist within CenterWell Home Health, responsible for assessing patients in their homes, developing and implementing therapeutic retraining programs to improve function, coordinating with physicians and the interprofessional team, designing adaptive equipment and community reintegration activities, supervising staff and documenting patient progress, and contributing to discharge planning and care coordination. Includes delivering rehabilitation-focused therapy to improve strength, coordination, range of motion, and independence, with emphasis on individualized Plan of Care and measurable outcomes.
