Job Summary
Make home visits to clients in designated geographic territories in Lamoille and Washington County, VT. Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, and ADL/IADL performance. Assess cognitive, psychological, and motor aspects to develop individualized intervention plans and evaluate home environments for hazards. Identify equipment needs and environmental adaptations, then continually revise care plans while educating patients and family members on rehabilitative care. Accurately document observations and interventions using a state-of-the-art touch pad tablet as an integral member of a multi-disciplinary team providing skilled nursing and rehabilitative care.
Required Qualifications
- A current Vermont Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- A minimum of one year of recent work experience as an Occupational Therapist
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
