Job Summary
Conduct home health visits to adult and geriatric clients in Orleans and Lamoille County, VT, performing thorough evaluations of functional status, muscle function, and ADL/IADL performance. Assess cognitive, psychological, and motor aspects to develop individualized intervention plans, while evaluating home environments for hazards and identifying necessary equipment adaptations. Continually revise care plans with nursing staff and educate patients or family members on rehabilitative care and safety strategies. Accurately document all observations and interventions using a state-of-the-art touch pad tablet. Requires a current Vermont Occupational Therapist license, one year of recent OT experience, and proficiency with electronic medical records.
Required Qualifications
- A current Vermont Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- A minimum of one year of recent work experience as an Occupational Therapist
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
