Job Summary
Conduct home visits to adult and geriatric patients in Chester County to perform functional evaluations, assess cognitive and motor performance, and evaluate home environments for safety hazards. Develop individualized intervention plans, identify equipment needs, and revise care plans in collaboration with nursing staff. Educate patients and family members on rehabilitative activities and accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania Occupational Therapist license and one year of recent OT experience.
Required Qualifications
- A current Pennsylvania Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Organizational skills
- Problem solving skills
- Solid computer skills
