Job Summary

Conduct home health visits to adult and geriatric patients in Vermont, performing thorough evaluations of functional status, muscle function, and cognitive aspects to develop individualized intervention plans. Assess home environments for hazards and identify equipment needs to enhance client independence. Continually revise care plans, collaborate with nursing on multidisciplinary care, and educate patients or family members on rehabilitative care. Accurately document all observations and interventions using a touch pad tablet. Requires a current Vermont Occupational Therapist license, one year of recent OT experience, and ability to work independently. Base pay ranges from $45.00 to $51.00 per point.

Required Qualifications

  • A current Vermont Occupational Therapist license
  • A minimum of one year of recent work experience as an Occupational Therapist
  • Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
  • Ability to work independently
  • Ability to manage time effectively

  • Solid computer skills

Desired Qualifications

  • Prior home care experience
  • Prior experience with electronic medical records (EMR)
