Job Summary

Evaluate guests and residents, develop individualized plans of care, and provide therapy interventions supporting activities of daily living, functional skills, and independence. Complete occupational therapy evaluations using appropriate measures, establish compliant care plans, and manage transitions between locations including equipment needs and handoff communication. Educate guests, residents, and families using clear communication principles, supervise support personnel, and coordinate care across Hillcrest Health & Rehab and Glen Haven Village. Identify individuals needing additional services, manage patient schedules, and address facility risks related to safety and infection control.

Required Qualifications

  • Graduate of an accredited Occupational Therapy program
  • Active Occupational Therapist license in Nebraska or a compact state
  • Basic Life Support certification through the American Heart Association
  • Must be able to speak, read, and write English
  • Must be able to independently lift up to 35 pounds
  • Must be able to perform repetitive sitting, bending, standing, walking, reaching, and movement as needed
  • Must be able to travel between Hillcrest Health & Rehab and Glen Haven Village as scheduled
  • Possible exposure to communicable diseases and bodily fluids

Desired Qualifications

  • At least two years of Occupational Therapy experience in long-term care, post-acute care, hospital, home care, or a similar setting
  • A passion for helping guests and residents improve function, safety, confidence, and independence
  • Strong evaluation, treatment-planning, and clinical-documentation skills
  • Strong communication and relationship-building skills with guests, residents, families, providers, and team members
  • Ability to lead plans of care while collaborating with assistants, rehab techs, and interdisciplinary partners
  • Strong attention to detail with documentation, safety, care coordination, and follow-through
  • Ability to manage multiple patient needs while maintaining high-quality care
  • Ability to stay organized and communicate effectively while supporting two locations
  • A calm, flexible, and professional approach in changing or stressful situations
  • Comfort using technology, clinical systems, and therapy resources efficiently and effectively
  • Dependable transportation
  • Previous long-term care, post-acute care, hospital, home care, or similar experience
