Job Summary
Provide skilled occupational therapy services in patients' homes on an intermittent basis, including evaluating function, developing and implementing interdisciplinary patient care plans with the physician, guiding patients in therapeutic self-care and activities to improve independence, instructing patients and families on use of wheelchairs, braces, splints, and prosthetics, and preparing progress notes and discharge summaries while coordinating with the clinical team and scheduling outpatient services as needed.
Required Qualifications
- Must be a graduate of an Occupational Therapy school accredited by ACOTE and eligible to take the NBCOT exam
- Must be licensed or registered by the state(s) of New Jersey/New York
- Must have at least one year of institutional experience
- Must have a criminal background check
- Must have current CPR certification (online not accepted)
