Job Summary
Conduct assessments and develop person-centered care plans for incoming and active participants. Serve as a member of the Multidisciplinary Team, assisting with carrying out the Team's plan of care. Provide Activities of Daily Living (ADL) care, including grooming, dressing, and hygiene, while training staff on proper techniques. Perform in-home evaluations for safety considerations and demonstrate safe practices using universal precautions. Supervise participants throughout the Center to ensure their safety and comfort. Attend all staff meetings, trainings, and in-services. This part-time role at a community-based nonprofit in Mountain View, CA, supports a mission-driven organization dedicated to improving patient outcomes and enhancing the quality of life for participants.
Required Qualifications
- Graduated from an accredited Occupational Therapy program
- Must have a current California state license as a Registered Occupational Therapist (OT)
- Must have a valid California Driver's license
- Must have a clean driving record
- Must have appropriate insurance
- Must have a reliable vehicle
- Must be able to cater to tasks with physical demands (lift, transfer, push/pull, maneuver, reposition, etc.)
- Must be able to safely move upwards of 35 pounds
