Job Summary
Conduct home visits in Manhattan to assess occupational therapy needs using objective diagnostic tests and evaluate therapeutic, rehabilitative, and functional status. Develop a comprehensive plan of care with measurable short and long-term goals, then provide treatment in collaboration with the patient's physician while documenting responses and modifying objectives as needed. Instruct patients, families, and healthcare personnel on safe mobility management and restoration techniques. Complete case conferences, discharge summaries, and weekly visit notes within seven days, while participating in interdisciplinary team interventions and quality assurance programs. Submit written home exercise programs and report any compliance violations to supervisors or the compliance hotline.
Required Qualifications
- Graduate of an Occupational therapy school approved by: The American Occupational Therapy Association; The Council on Medical Education and American OT Association of the Council on Medical Education of Hospitals for the American Medical Associate.
- A License and current registration to practice as a Registered Occupational Therapist in New York State.
- Two (2) years institutional and/or home care experience required.
- Satisfy the state health status requirements.
