Job Summary
Occupational Therapist needed to evaluate and treat geriatric patients across multiple assisted living locations (Concord, Manchester, Bedford). Responsibilities include conducting patient evaluations of functional abilities, ADLs, and cognitive performance; developing and implementing individualized care plans focused on ADLs, functional mobility, and other patient-centered goals; leading wellness initiatives and group activities; collaborating with an interdisciplinary team (PTs, SLPs, nursing) to optimize outcomes; educating patients and families on strategies to improve independence; documenting care in EMR compliant with state/facility standards; delivering care at one or more locations with a focus on high-quality, evidence-based practice. Requirements include graduation from an accredited OT program, active or pending state OT license, passion for quality patient care, excellent communication, and interest/experience in geriatrics with skills in adaptive equipment and ADL training. Required Qualifications
- Graduate of an accredited occupational therapy program
- Active or pending state license to practice occupational therapy
- Experience or interest in geriatrics
- Strong communication and interpersonal skills
- Ability to work with patients, families, and colleagues
- Adaptive equipment and ADL training skills
