Job Summary
Provide occupational therapy assessments and interventions to improve patients' functional abilities in their homes. Develop individualized treatment plans based on assessments, monitor progress, and adjust plans as needed. Collaborate with other healthcare professionals to coordinate patient care and educate patients and families on therapeutic techniques. Requires a Master's degree in Occupational Therapy and a valid state OT license; experience in case management and home health or geriatric/post-acute care is preferred.

- Valid state license or certification as an Occupational Therapist
