Job Summary
Occupational Therapist (OT) providing person-centered care to improve residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes. Responsibilities include consulting with patients, evaluating physical conditions, developing individualized treatment plans to maximize ADL/IADL performance, recommending home/environment modifications and adaptive equipment, delivering treatments per care plans, collaborating with the interdisciplinary team, documenting thoroughly in compliance with policies, and attending required meetings. Must hold an active Texas OT license; SNF/long-term care experience and Net Health EDS familiarity preferred. Strong communication and clinical decision-making skills are essential.
Required Qualifications
- Active and valid Texas Occupational Therapy license
- In good standing with all regulatory agencies and licensing boards
- Skilled Nursing Facility (SNF) or long-term care experience preferred
- Current knowledge of occupational therapy treatment practices
- Experience with Net Health Electronic Documentation System preferred
- Ability to manage patients with varying personalities and needs
- Strong communication, collaboration, and clinical decision-making skills
