Job Summary
Direct, supervise, evaluate and provide occupational therapy services to patients in the home as prescribed by the attending physician. Essential duties include instructing patients and families in activities of daily living, establishing household management routines, instructing in the use of prosthetic, orthotic and assistive devices, educating patients and caregivers in adaptive equipment, managing falls risk through evaluation and care planning, recording and reporting patient responses to the OT program, communicating with other health team personnel, preparing clinical/progress notes on the day of visits for inclusion in the clinical record, attending case conferences and staff meetings, and directing or assisting with the management of home health aide service delivery. Other duties as assigned. Schedule is Mon - Fri Days with 40 hours per week. Education: Graduate of an accredited Occupational Therapy program. License: Current, valid OT license issued by NYS Department of Education. Experience: One year of practical experience as a licensed occupational therapist. Environment: Primarily non-office setting; potential exposure to outdoor conditions; medium work. EOE/AA Minority / Female / Disability / Veteran.
Required Qualifications
