Job Summary
Provide one-on-one Occupational therapy to older adults in private residences, senior living communities, and memory care settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment to support patient mobility, independence, and quality of life. Document sessions effectively within FOX's EMR system while collaborating with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging in place, incorporating telehealth and virtual care when appropriate to extend access and flexibility. Manage your own treatment schedule with autonomy while meeting physical demands for patient transfers, lifting, and extended periods of standing or walking.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Ability to wear PPE when required
