Job Summary
Undertake holistic, person-centred Occupational Therapy assessments for adults with complex health needs, managing a caseload of approximately four cases per week. Assess for and recommend specialist equipment, manual handling solutions, and major adaptations to ensure safe hospital discharges and maintained independence. Complete support planning and reviews for adults and their carers while collaborating with health, housing, and social care professionals. Provide advice and training on the safe use of equipment in line with legislation and best practice. Maintain accurate case records and contribute to safe discharge planning. HCPC-registered Occupational Therapist with extensive experience in complex assessments and multidisciplinary team work required.
Required Qualifications
- HCPC-registered Occupational Therapist
- Extensive experience assessing for complex equipment, specialist equipment, manual handling, and major adaptations
- Experience working within a multidisciplinary team undertaking complex Occupational Therapy assessments
- Strong knowledge of Adult Social Care legislation and housing legislation
- Proven ability to independently manage, prioritise, and progress complex cases
- Experience reviewing equipment provision and educating service users and carers on safe use
- Excellent communication, organisational, and clinical reasoning skills
