Job Summary
Occupational Therapist needed to conduct therapy evaluations and develop/implement treatment plans for home-based hospice patients. Responsibilities include evaluating physical, cognitive, and functional status; collaborating with the multidisciplinary team to form and communicate the plan of care; providing patient/family education; teaching home exercise programs; managing caseload and coordinating services; documenting assessments and communications; and coordinating follow-up after discharge. Requires licensing as an OT in California, BLS certification, and 1 year of relevant experience; opportunities for supervision and education of staff and students, with a compensation range of $49.91-$61.66 per hour (annualized to $103,813–$128,253) and comprehensive benefits.
Required Qualifications
- California Occupational Therapist License (Vendor Managed)
- National Provider BLS - American Heart Association (Vendor Managed)
- Education: Bachelor's or Masters in Occupational Therapy; Ph.D. preferred in lieu of Bachelor’s/Master’s; 1 year experience in acute care, rehabilitation, or home care
