Job Summary
Conduct 1:1 occupational therapy with patients to support their aging-in-place objectives across Crawford, Mercer, Venango, Erie, Forest, Warren, Butler, and Lawrence counties. Deliver skilled nursing, palliative care, and comprehensive therapy services via in-person visits or remote/video capabilities. Maintain a licensed status in Pennsylvania and ensure reliable transportation for home visits. Full-time and part-time roles available with immediate impact opportunities.
Required Qualifications
- Licensed as an Occupational Therapist in the Commonwealth of Pennsylvania
- Reliable, independent means of transportation to make home visits
- Valid driver's license
- Car insurance
Desired Qualifications
- Minimum of 6 months acute care or rehabilitation center experience preferred
