Job Summary
Evaluate patients, develop progressive treatment programs, and administer therapy services per the Plan of Care. Administer appropriate evaluations to assess function, set goals, and document treatment daily in medical charts. Supervise COTAs and HHAs, coordinate total patient care, and assist families with home programs and adaptive equipment. Submit visit records, discharge summaries, and referrals for paramedical services while maintaining HIPAA confidentiality. Paid per visit starting at $80, with flexible scheduling and mileage reimbursement. Requires a licensed OT with one year experience preferred.
Required Qualifications
- Licensed OT
- Valid driver's license
- Ability to attend an orientation at our Willowbrook office for 1-3 days
Desired Qualifications
- Minimum of 1 year as an OT experience preferred
- OASIS and Wellsky (Kinser) experience strongly preferred
- Physical within the last year
- Current TB results (within last 12 months) and CPR Certification
