Occupational Therapist

Deliver high-quality, in-home therapy for seniors and adults with chronic conditions by performing comprehensive assessments, developing and implementing individualized treatment plans, and focusing on improving patients' safety, independence, and quality of life at home. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care using electronic systems within a supportive, patient-first culture.
