Occupational Therapist

Conduct comprehensive assessments and develop individualized treatment plans for seniors and adults with chronic conditions. Deliver high-quality in-home therapy focused on improving patients' safety, independence, and quality of life. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care using electronic systems within a supportive, patient-first culture. This role offers PRN flexibility for CenterWell Home Health.
