Job Summary
Coordinate, supervise, and coach Community Support Workers while conducting professional patient assessments in homes and the community. Develop effective Care Plans with Ontario Health atHome, collaborate with interdisciplinary teams, and establish measurable goals with patients. Provide one-on-one therapeutic support to promote client independence in the home and community while offering active guidance on workplace Health and Safety. This role supports CarePartners' expansion of its rehabilitation program across Stratford, ON, serving approximately 72,000 patients annually. Therapists operate under a self-scheduling model with flexible hours, earning a per-visit rate between $74.48 and $86.85.
Required Qualifications
- Master's Degree (or equivalent) degree in Occupational Therapy from a recognized university
- Registration with the College of Occupational Therapists of Ontario
- Proven computer skills, including email, MS Word and Outlook, work scheduling systems
- Strong assessment skills and the ability to participate in effective treatment programs
- Provide one-on-one therapeutic support with clients
- Promotes independence of the client in the home and in the community
- A reliable vehicle for some regular travel
- Excellent English verbal and written communication skills
- Clear Background and Vulnerable Sector Check
Desired Qualifications
- French language skills are an asset
