Job Summary
Conduct participant-centered evaluations, treatments, and interventions to maintain independence, safety, and quality of life at home and in the community. Develop treatment programs, set goals, and provide occupational therapy as determined by assessments. Document initial and reassessments of functional status, review cases for discharge, and assess needs for adaptive and durable medical equipment. Coordinate with the Home Care Coordinator for equipment purchases, communicate participant changes to the Interdisciplinary Team, and participate in care conferences. Complete home visits when directed and provide training to family members and staff on treatment plan components.
Required Qualifications
- Active and unencumbered Occupational Therapy license in the state of Michigan
- Two (2) years' experience as an OT
- Valid Driver's license
- Reliable transportation
Desired Qualifications
- CPR certification or willing to obtain
