Job Summary
Provide Occupational Therapy services to PACE participants across the PACE center, clinic, participants’ homes, and contracted facilities. Responsibilities include conducting initial and periodic in-person assessments, developing and revising individualized treatment plans, delivering OT interventions, educating participants and caregivers on equipment use and home safety strategies, coordinating adaptive equipment and home modifications, maintaining timely documentation, participating in daily IDT care planning, supervising Certified Occupational Therapy Assistants, training staff on OT practices, and ensuring compliance with regulatory standards. Maintains licensure and engages in ongoing professional development; supports quality improvement and utilization review; may perform dual duties as Home Care Coordinator as needed. Requires an advanced degree in OT, current state licensure, and experience in interdisciplinary teams; prior experience with frail or elderly populations preferred.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
- Pass a comprehensive criminal background check including federal/state Medicare/Medicaid exclusion lists, license verification, and drug screen
