Job Summary
Occupational Therapist for BoldAge PACE responsible for planning, organizing, and implementing OT services across PACE settings, including assessments, treatment planning, and caregiver/participant education. Duties include in-person initial and periodic assessments of functional abilities and home safety, coordinating adaptive equipment and home modifications, developing and revising individualized treatment plans with IDT collaboration, delivering OT interventions at the PACE center, clinics, participants’ homes, and contracted facilities, educating participants and caregivers on equipment use and home safety, maintaining documentation per standards, participating in daily IDT meetings, supervising COTAs, training staff in safe OT practices, and supporting quality improvement and compliance initiatives. Requires current OT licensure in the state, a relevant advanced degree (master’s level), and 2 years of interdisciplinary team experience (preferably with frail/elderly populations). Typical schedule is Monday–Friday, 8:00am–4:30pm (40 hours/week) with location in Palos Heights, IL.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
- 1 year of experience working with a frail or elderly population preferred.
