Job Summary
Occupational Therapist responsible for planning, organizing, and implementing OT services for PACE participants as part of the interdisciplinary team. Duties include initial and periodic in-person assessments of functional abilities and home environment, developing individualized treatment plans, delivering OT interventions across settings (PACE center, clinic, participants’ homes, contracted facilities), educating participants and caregivers on equipment use and safety, maintaining accurate documentation, participating in IDT meetings, supervising COTAs, and training staff in safe OT practices. Role may encompass Home Care Coordinator duties and requires current OT licensure, ability to travel, and adherence to regulatory standards; 40 hours/week, Monday–Friday. Location: Argo Fay, IL.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience preferred in interdisciplinary team on a hospital, nursing home or community-based setting
- 1 year of experience with frail or elderly population preferred
