Job Summary
Occupational Therapist at BoldAge PACE focuses on planning, organizing, and delivering OT services to PACE participants as part of the interdisciplinary team. Responsibilities include initial and periodic in-person assessments of participants’ functional abilities and home environment, recommending and coordinating adaptive equipment and home modifications, developing and revising individualized treatment plans, delivering OT interventions across settings (PACE center, clinic, participants’ homes, contracted facilities), educating participants and caregivers on equipment use and safety, maintaining timely documentation, participating in daily IDT meetings, supervising COTAs, mentoring staff, and potentially performing duties of other IDT members to ensure cohesive care. Requires an Advanced OT degree, current state license, and strong experience in interdisciplinary settings; 40 hours per week, Monday–Friday, La Grange, IL.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
- 1 year of experience working with a frail or elderly population preferred.
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
- Pass a comprehensive criminal background check that may include federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
- All Employees - Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
