Job Summary
Occupational Therapist position with BoldAge PACE. Plans, organizes, and implements Occupational Therapy services for PACE participants as part of the interdisciplinary team. Conducts initial and periodic in-person assessments of functional abilities and home environment; develops and revises individualized treatment plans; delivers OT interventions across the PACE center, clinic, participants’ homes, and contracted facilities; educates participants and caregivers on equipment use, home safety, and ADL programs; maintains timely documentation; actively participates in daily IDT meetings; supervises COTAs including review of care plans and co-signing documentation; provides mentorship and staff training on OT practices; may perform duties of other IDT members as needed; supports quality improvement and compliance initiatives; maintains licensure and ongoing professional development; requires advanced degree in OT, state licensure, and at least two years of interdisciplinary team experience; schedule is Monday–Friday, 8:00am–4:30pm (40 hours/week); location: Berkeley, IL; strong emphasis on compassionate geriatric care and home-based support.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring (If applicable for the role)
- Competency position specific competencies for the Occupational Therapist will be met prior to assuming participant care
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
- Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen
