Job Summary
Plans, organizes, and implements Occupational Therapy services for PACE participants as part of the interdisciplinary team. Conducts initial and periodic in-person assessments of participants’ functional abilities, home environment, and rehabilitation needs; develops, implements, and revises individualized treatment plans; delivers OT interventions across settings including the PACE center, clinics, participants’ homes, and contracted facilities; educates participants and caregivers on equipment use, home safety strategies, and ADL programs; maintains timely documentation and participates in daily IDT meetings; supervises Certified Occupational Therapy Assistants and mentors staff in safe OT practices; may perform duties of other IDT members to ensure cohesive planning; contributes to quality improvement, utilization review, and compliance initiatives; maintains licensure and ongoing professional development; requires an Advanced degree in OT and current state license; typically 40 hours/week, Mon-Fri, at Park Forest, IL.
Required Qualifications
- Advanced degree in Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience on an interdisciplinary team preferable
