Job Summary
Plans, organizes, and implements Occupational Therapy services for PACE participants as a member of the PACE interdisciplinary team. Responsibilities include assessment, planning, providing treatment, and teaching participants, caregivers or family to maintain participant support in the community. Conducts in-person assessments of functional abilities and home environment; recommends adaptive equipment, home modifications, and therapy services; develops and revises individualized treatment plans; delivers OT interventions across the PACE center, clinic, participants’ homes, and contracted facilities; educates participants and caregivers on equipment use and home safety and ADL programs; maintains timely documentation per standards; participates in IDT meetings; supervises COTAs and mentors staff; may perform duties of other IDT members; supports quality improvement and compliance initiatives; maintains licensure and ongoing professional development.
Required Qualifications
- Advanced degree in Occupational Therapy
- Current license to practice Occupational Therapy in the state of Illinois
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting (preferred)
- 1 year of experience working with a frail or elderly population (preferred)
- Be medically cleared for communicable diseases and have all immunizations up to date
- Pass a comprehensive criminal background check (may include federal/state Medicare/Medicaid exclusion lists, license verification, reference checks, drug screen)
- Reliable transportation and driver's license with minimum state-required auto insurance
