Job Summary
Occupational Therapist at BoldAge PACE plays a key role on the interdisciplinary team, planning, organizing, and implementing OT services for participants. Responsibilities include performing initial and periodic in-person assessments of functional abilities and home environment, recommending adaptive equipment and home modifications, developing and revising individualized treatment plans, delivering OT interventions across settings (PACE center, clinic, participants’ homes, contracted facilities), educating participants and caregivers on equipment use and safety, maintaining accurate documentation, participating in IDT meetings, supervising COTAs, training staff in safe OT practices, and supporting quality improvement and compliance initiatives. May perform responsibilities of other IDT members as needed, with licensure and experience guiding scope. Requires advanced degree in OT and current state licensure; 2 years of interdisciplinary team experience preferred and 1 year of frail/elderly population experience preferred; reliable transportation and medical clearances are required; background checks may be conducted.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring (If applicable for the role)
- Competency position specific competencies for the Occupational Therapist will be met prior to assuming participant care
- Be medically cleared for communicable diseases and have immunizations up to date before engaging in direct participant contact
- Pass a comprehensive criminal background check that may include federal and state Medicare/Medicaid exclusion lists, criminal history verification, license verification
- Maintain current licensure, certifications, and participation in ongoing professional development
