Job Summary
Occupational Therapist needed to plan, organize, and implement OT services for PACE participants as part of an interdisciplinary team. Responsibilities include assessment, treatment planning, hands-on OT interventions across settings (PACE center, clinic, participants’ homes, contracted facilities), educating participants and caregivers, supervising COTAs, maintaining documentation, participating in IDT meetings, staff training, quality improvement, and ensuring licensure/certifications are current. Requires advanced OT degree, state licensure, 2+ years in an IDT setting (preferable), reliable transportation, immunizations, background check, and flexible 40-hour workweek (Mon-Fri, 8:00am-4:30pm) in Peotone, IL. Matches include helping seniors live independently, home safety and ADL support, and collaboration across care settings.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- Pass a comprehensive criminal background check
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
- All Employees - Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
- Evidence of immunizations up to date
- Competency position specific competencies for the Occupational Therapist will be met prior to assuming participant care
