Job Summary
Occupational Therapist at BoldAge PACE responsible for planning, organizing, and delivering OT services across the PACE continuum, including assessments of participants’ functional abilities and home environment; developing and revising individualized treatment plans; delivering interventions in the PACE center, clinic, homes, and contracted facilities; educating participants and caregivers on equipment use and safety; supervising COTAs and mentoring staff; participating in IDT care planning and quality initiatives; and maintaining licensure and ongoing professional development. Requires current OT license in the state, an advanced degree in OT, and experience on an interdisciplinary team with preference for work with frail or elderly populations.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited program
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience on an interdisciplinary team preferred
- 1 year of experience with frail/elderly population preferred (training provided if not present)
- Reliable transportation and valid driver's license with appropriate auto insurance
- Background check and pre-employment screenings
