Job Summary
Occupational Therapist at BoldAge PACE responsible for planning, organizing, and implementing OT services across PACE settings (center, clinic, home, and contracted facilities). Conduct initial and periodic in-person assessments of participants’ functional abilities and home environment; develop and revise individualized treatment plans; administer OT interventions; educate participants and caregivers on equipment use and home safety; supervise Certified Occupational Therapy Assistants (COTAs) and mentor staff; participate in daily IDT meetings for comprehensive care planning; ensure accurate documentation; support quality improvement and compliance initiatives; maintain licensure and pursue ongoing professional development. Requires an advanced degree in Occupational Therapy and current state license; minimum of 2 years on an interdisciplinary team preferred, with 1 year of experience serving frail or elderly populations preferred. Schedule is Monday–Friday, 8:00am–4:30pm, 40 hours per week, Willow Springs, IL.
Required Qualifications
- Advanced degree in Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
