Job Summary
Occupational Therapist needed to plan, organize, and implement OT services for PACE participants as part of an inter-disciplinary team. Responsibilities include conducting in-person assessments, developing and revising individualized treatment plans, delivering OT interventions across the PACE center, clinic, and participants’ homes, educating participants and caregivers on equipment use and safety, supervising COTAs, ensuring accurate documentation, participating in IDT meetings, and supporting quality improvement and compliance initiatives. Must maintain current OT licensure and engage in ongoing professional development; position includes typical 40-hour workweek and benefits.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience preferable on an interdisciplinary team in hospital, nursing home or community-based setting
- 1 year of experience with frail or elderly population preferred (training provided if not present)
- Reliable transportation and valid driver’s license with state-required auto insurance
- Criminal background check and compliance requirements
- Continued professional development and licensure maintenance
