Job Summary
Occupational Therapist for BoldAge PACE: plans, organizes, and implements OT services for participants as part of the PACE interdisciplinary team. Conducts initial and periodic in-person assessments of functional abilities and home environment, develops and revises individualized treatment plans, delivers OT interventions across the PACE center, clinic, homes, and contracted facilities, educates participants and caregivers on equipment use and home safety, maintains documentation, participates in IDT meetings, and supervises COTAs with mentorship. May perform Home Care Coordinator duties and supports quality improvement, utilization review, and compliance activities. Requires current OT license, master's degree in OT, and 2+ years of interdisciplinary-team experience; standard pre-employment requirements apply. 40 hours per week, Monday–Friday, Glenwood, IL.
Required Qualifications
- Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring (If applicable for the role)
- Competency position specific competencies for the Occupational Therapist will be met prior to assuming participant care
- All Employees - Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
- Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen
