Job Summary
Occupational Therapist, plans, organizes, and implements OT services for PACE participants as part of the interdisciplinary team. Conducts initial and periodic in-person assessments of functional abilities and home environment; recommends adaptive equipment and home modifications; develops and revises individualized treatment plans; delivers OT interventions across the PACE center, clinic, participants' homes, and contracted facilities; educates participants and caregivers on equipment use and home safety; maintains documentation per standards; participates in daily IDT meetings; supervises COTAs and reviews care plans; trains staff in safe OT practices; may perform duties of other IDT members to ensure cohesive care; supports quality improvement and compliance initiatives; maintains licensure and ongoing professional development; typical schedule Monday–Friday, 8:00am–4:30pm, 40 hours/week; Location: Illinois (US).
Required Qualifications
- Advanced degree in Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
- Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen
